Integrate into html?

How would I integrate imagevue into a HTML page? What lines of code would I use?
Below are a few examples of integrating Imagevue into a HTML document:

Keep in mind they are created for an earlier version of Imagevue, but you can use the same codes to integrate the latest imagevue.swf file(or any other SWF).
